About the practice

Thoughtful planning, not crisis management

The Later Life Navigation practice provides compassionate, expert guidance for adults in later life and the people who care about them — in partnership with SuperNurse NZ. Most people hope to remain living independently at home for as long as possible, and every journey is unique. Sometimes additional support at home is the right answer; sometimes a move into residential care becomes the safest, most positive next step. Neither is a success or a failure — what matters is that decisions are made with knowledge, preparation and respect for the person’s wishes, values and quality of life.

Our approach begins by listening. We take time to understand the individual — their story, their strengths, their concerns, and what matters most to them — then translate complex health information into clear, practical guidance so people and families can make informed decisions with confidence.

Clear advice when it matters most

Many older people and families feel overwhelmed when changes begin to occur. Waiting lists, confusing systems and uncertainty can leave people feeling lost. Our role is to provide clear advice, practical support and a plan that helps people make informed decisions about the future. When there isn’t time to plan, we also provide short-term clinical support and coordination during a crisis.

Lyndsey Lloyd, RMHN

Our practice was developed by Lyndsey Lloyd, Registered Mental Health Nurse, whose career spans more than 25 years supporting people experiencing mental illness, dementia, cognitive change and the complexities of ageing. After moving from the United Kingdom to Aotearoa New Zealand, Lyndsey returned to frontline nursing to better understand the realities facing people, families and health professionals in our local communities — reinforcing a simple belief: too many families are left navigating later life without the guidance and support they need.

Everything we do is underpinned by compassion, dignity, partnership, integrity and evidence-based nursing practice.
